How should I organize lab results so another care team can read them?

Keep each lab result with its test name, value, unit, reference range, collection date, reporting date, and source lab or hospital.

Do not merge values into a new table unless the original reports stay attached. A structured packet made manually or with MedDossier should preserve the source report and show any trend table as a secondary view.
